I. OBJECTIVE AND PURPOSE

This policy establishes the process by which applicants to the University of North Florida (UNF) provide medical documentation to comply with immunization requirements for admission and provides enforcement mechanisms to achieve compliance for UNF students.

II. STATEMENT OF POLICY

  1. Medical History
    1. All applicants or students must submit documentation of required/recommended immunizations through their student patient portal prior to course registration.
    2. Medical documents must include the applicant or student’s full name and date of birth. Example of acceptable medical documentation includes:
      1. Student Health Services (SHS) Medical Compliance Immunization Form;
      2. Electronic medical records;
      3. Health Department records;
      4. High School transcripts;
      5. Military records;
      6. Previous college/institution records;
      7. Medical records from healthcare provider;
      8. Titer lab report;
      9. International vaccine records; OR
      10. Vaccine booklet from childhood.
  2. Proof of Immunity
    1. Measles, Mumps, and Rubella
      1. Required for anyone born after Dec. 31, 1956. Documentation must demonstrate TWO DOSES. The first MMR must have been given on or after January 1, 1968, and on or after the first birthday*. The second MMR immunization must have been given 28 days or more after the first MMR.; OR
      2. Provide laboratory (serologic) evidence of immunity by having a blood test to check for antibodies for Measles, Mumps and Rubella. Laboratory results must include the applicant or student’s name and date of birth and show POSITIVE immunity to each of the three diseases. An equivocal or negative titer on any of the three tests will require the applicant or student to receive the MMR series. * According to Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC), vaccine doses administered ≤4 days before the minimum interval or age are considered valid (known as the “grace period”).
    2. Hepatitis B
      1. Medical documentation of immunization of Hepatitis B vaccine; OR
      2. Copy of laboratory (serologic) evidence of Hepatitis B immunity; OR
      3. Signed declination (waiver of liability) of the vaccine.
    3. Meningitis
      1. Medical documentation of immunization with Meningitis vaccine at age 16 or later; OR
      2. Signed declination (waiver of liability) of the vaccine.
  3. Exemptions
    1. Medical: Medical exemptions must be from a healthcare provider on office/provider letterhead or official form, signed, and dated. The documentation must be uploaded to the student patient portal.
    2. Religious: Religious exemptions include a letter from a religious official or lawyer on office letterhead, official forms (for example: Health Department form), or documentation from a previous school attended. The documentation must be signed and dated. The documentation must be uploaded to the student patient portal. In lieu of documentation, the applicant or student may complete the electronic religious/sincerely held belief exemption under the Forms tab of the patient portal.
    3. Distance Learning: Students enrolled in Distance Learning programs as identified by the University are exempt from the requirement to show proof of immunizations. If the student enrolls in on-campus courses, the student must comply with immunization requirements prior to the first day of class.
    4. Special Circumstances
      1. Pregnancy/Fertility Treatments: On dated office/provider letterhead, specify the student’s name, date of birth, and expected date of delivery (if pregnant). The documentation should include a brief explanation of the circumstances and contact information of the provider. The documentation must be signed, dated, and from someone who is not related to the applicant or student. The documentation must be uploaded to the student patient portal.
      2. Breastfeeding/Lactation Considerations: Excretion of Measles, Mumps and Rubella (MMR) in breast milk unknown; applicants or students considering this exemption should discuss receiving the MMR vaccine with their provider. The documentation must be on dated office/provider letterhead, specify the student’s name, date of birth, provide a brief statement that the student is breastfeeding, and that the provider does not support receipt of the MMR vaccine. The documentation must be signed, dated, and from someone who is not related to the applicant or student. The documentation must be uploaded to the student patient portal.
    5. In the event of an outbreak, exempted students will be excluded from all campus activities until such time as is specified by the County Health Department.

II. VIOLATIONS

Students who fail to comply with immunization requirements as stated above may be referred to the Dean of Students Conduct Team, restricted or removed from on-campus housing assignment, receive a registration hold until compliance is achieved, or be subject to other appropriate restrictions or compliance efforts.
